OBJECTIVES: The presence of solitary median maxillary incisor (SMMCI) is a rare abnormality of craniofacial development that can be associated to other anomalies of median line, being holoprosenfaly a form of lower expression. The SMMCI has an hereditary autosomal dominant transmission with incomplete penetrancy and a high variability phenotype in familiar cases (Tabatabaie et al., 2008; Bolan et al., 2009). Is objective of this work to present a clinical case and treatment plane of a male patient, that have a solitary median maxillary incisor evaluated by Orofacial Genetic Department and Orthodontic Department of Dental Faculty of University of Oporto.
MATERIALS AND METHODS:The patient was studied following a protocol that included clinical examination, radiographic and photographic records, preparation of study models and assembly in semi-adjustable articulator. Were requested auxiliary diagnostic tests, consisting in analytic and imaging exams. It was additionally performed a literature search in PUBMED with the keywords "solitary median maxillary central incisor" and limited to Portuguese, English and Spanish language and to the years 2000 and 2010 . This search included 18 articles associated with the theme.
RESULTS: The patient RHLR, Caucasian, gender male, with 13 years old presented facial asymmetry, hypotelorism and convergent strabismus. He was a severe dolichofacial, with short base of cranium. Of orthodontic point of view and in agreement with the literature about the treatment of SMMCI, the absence of inter-nasal suture and part of inter-maxillary suture has no indication to a transversal palatine expansion. In some of cases could be indicated, the extraction of the solitary central incisor and the mesialization of lateral incisors.
CONCLUSIONS: The presence of solitary central maxillary incisor should not be regarded as a simple dental anomaly of development. It requires a medical evaluation performed by a multidisciplinary team, in order to establish the most appropriate treatment plan for the patients.
