Paraneoplastic sclerodermiform syndrome--case report.

Rovisco, J; Serra, S; Abreu, P; Coutinho, M; Santiago, T; Inês, L; Pereira da Silva, JA

Occasionally, auto-immune diseases may emerge as paraneoplastic syndromes. This is especially recognized in the case of polymyositis/dermatomyostis, but it is an extremely rare event in systemic sclerosis (SSc). The authors report the case of a sixty-year-old woman who presented with Raynaud's phenomenon and rapidly progressing skin thickness of the forearms, hands and lower limbs. Patient evaluation revealed a...

Identification of clinical predictors of flare in systemic lupus erythematosus ...

Inês, L; Duarte, C; Silva, RS; Teixeira, AS; Fonseca, FP; Pereira da Silva, JA

Objective. SLE has a relapsing-remitting course with disease activity flares over time. This study aims to identify clinical predictors of SLE flares.Methods. This prospective cohort study over 24 months included all SLE patients on follow-up at one academic lupus clinic. Flare was defined as an increase in SLEDAI-2K score ≥4 points. Baseline clinical and demographic parameters were compared using survival anal...

Tolerogenic versus Inflammatory Activity of Peripheral Blood Monocytes and Dend...

Carvalheiro, T; Rodrigues, A; Lopes, A; Velada, I; Ribeiro, A; Martinho, A; Inês, L; Pereira da Silva, JA; Pais, ML; Paiva, A

Abnormalities in monocytes and in peripheral blood dendritic cells (DC) subsets have been reported in systemic lupus erythematosus (SLE). We aim to clarify the tolerogenic or inflammatory role of these cells based on ICOSL or IFN-α and chemokine mRNA expression, respectively, after cell purification. The study included 18 SLE patients with active disease (ASLE), 25 with inactive disease (ISLE), and 30 healthy c...

Functional characterization of peripheral blood dendritic cells and monocytes i...

Henriques, A; Inês, L; Carvalheiro, T; Couto, M; Andrade, A; Pedreiro, S; Laranjeiro, P; Morgado, JM; Pais, ML; Pereira da Silva, JA; Paiva, A

With the purpose of contributing to a better knowledge of the APCs functional activity in SLE, we evaluated the distribution and functional ability to produce pro-inflammatory cytokines (TNF-α, IL-1β, IL-6 and IL-12) of peripheral blood (PB) monocytes and DC (tDC), particularly myeloid (mDC) and CD14(-/low)CD16(+) DC subpopulations comparing them with those obtained from healthy individuals. The study was perfo...

Th17 cells in systemic lupus erythematosus share functional features with Th17 ...

Henriques, A; Inês, L; Pais, ML; Pereira da Silva, JA; Paiva, AA

This study was designed to investigate the functional heterogeneity of human Th17 and how their plasticity shapes the nature of immune cell responses to inflammation and autoimmune diseases, such as systemic lupus erythematosus (SLE). We evaluated functional Th17 cell subsets based on the profile of cytokine production in peripheral blood (PB), bone marrow aspirates (BM) and lymph node biopsies (LN) from health...

Oral contraceptives and systemic lupus erythematosus: what should we advise to ...

Duarte, C; Inês, L

Oral contraceptives (OC) are the contraceptive method of choice for the majority of Western world women. Decision on giving OC to patients with Systemic Lupus Erythematosus (SLE) puts special issues and concerns. In fact, OC have been evocated as etiologic risk factors for SLE and also associated with an increased risk of flares. During periods of active disease an effective contraception is mandatory, but OC p...

Linfoma difuso de grandes células em doente com lúpus eritematoso sistémico

Duarte, C; Couto, M; Inês, L; Silva, J; Sousa, I; Malcata, AB

The authors present the case of a 44-year-old female patient with Systemic Lupus Erythematosus diagnosed 4 years earlier. She presented with constitutional symptoms and back pain and one month later a diagnosis of Non Hodgkin Lymphoma was established (subtype Diffuse Large B Cells Lymphoma). The risk of malignancy associated to SLE is discussed.

Rash, fever and proteinuria after amoxicillin in a SLE patient

Couto, M; Duarte, C; Geraldes, A; Medeiros, C; Inês, L; Malcata, AB

We report a case of severe type IV hypersensitivity reaction to amoxicillin, which occurred in a person with a 12-year history of SLE. The present case illustrates the wide differential diagnosis in a SLE patient who presents with an allergic drug reaction. The attribution of the presenting symptoms to the underlying SLE and/or to the drugs used to treat SLE and coexisting conditions is a major challenge.

Perfil de risco cardiovascular numa população portuguesa de doentes com LES

Duarte, C; Couto, M; Vaz, C; Inês, L; Malcata, AB

Systemic Lupus Erythematosus is associated with an increased risk of atherosclerosis and cardiovascular events, as compared to the general population. Objective: To evaluate the prevalence of «traditional» cardiovascular risk factors in Portuguese SLE patients and to estimate the associated risk of cardiovascular events. To evaluate the needs for optimization of prophylactic measures to prevent cardiovascular e...
